Extended Day Registration Form
Hours of Extended Day:
Monday- Friday
Mornings: 7:00-8:00 a.m. - All students must be escorted into the school building by parent or guardian.
Afternoons: 3:40-6:00 p.m. - Parents will be charged $1.00 per minute for each minute they are late picking up their child(ren) beginning at 6:00 p.m., late fees must be paid at pick-up.

* $25.00 Non-Refundable Registration fee
* $35.00 Mornings Only weekly tuition
(Monday – Friday, 7-8 a.m.)
* $50.00 Afternoon Only weekly tuition
(Monday – Friday, 3:40p.m. - 6 p.m.)
* $75.00 Extended Day weekly tuition
Both Before and After School Program
Students who remain in the evenings receive a supper snack and 1st – 5th graders will have the option to begin their homework, however, it is not tutoring.
Payments must be made online by going to our website: http://www.audubonelementaryeagles.com.  Payments must be made by 8:00 AM on Tuesday of the participating week.  If payment is not received the student will not be allowed to remain in the program, until payment is made! The extended day program is a self-sustaining program, we must receive payments for the service to continue.
